Community Outreach Team (CREST) Club HEAL

The Community Outreach Team (CREST) serves as a mental health first-stop touchpoint. CREST promotes early identification of at-risk individuals and provides psychosocial intervention to support those with mental health needs and their caregivers in the community.  

This team works with other community partners to improve mental health and dementia literacy as well as conducting preventive activities to reduce the risk of mental health and dementia issues, such as cognitive stimulation activities.  

CREST is one of the programmes under the Community Mental Health Masterplan developed by the Agency for Integrated Care (AIC), together with the Ministry of Health (MOH), which enables persons living with mental health issues and dementia to seek early treatment nearer to their homes and ensure that they are well-supported in the community. 
 

How can the Community Outreach Team support you?

• Promote early recognition of signs and symptoms of mental health conditions and/or dementia.  

• Improve mental health conditions and/or dementia literacy by organising targeted outreach events and activities to provide mental health and/or dementia information and education to residents and caregivers in the region.

• Provide psychosocial intervention such as techniques to reduce the risk of escalation of mental health needs. 

• Provide collaborative and coordinated care for clients and caregivers through a case management approach. 

• Work in partnership with health and community care partners and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.
   

CREST serves individuals aged 18 and above who are Singapore citizens or Singapore permanent residents.
